* If you do not see your plan listed above, please don't hesitate to contact our office as
some insurance companies do offer out-of-network benefits.
Things to keep in mind:
* "Healthy New York" - Healthy New York plans do not allow for physical therapy unless:
-you are post-surgery
-you are post-hospitalization
* Authorization - Many insurance plans require authorization for physical therapy. Authorization is obtained upon your first visit. Although many plans allow for a certain amount of visits per year, how they are distributed is at the discretion of the authorization company.
* "Direct Access" - As of July 27th, 2006, New York became the 43rd state to pass a law allowing patients "direct access" care to any license Physical Therapist. Under this legislation, patients may see a Physical Therapist for up to ten (10) visits or 30 (thirty) days, whichever comes first, without a prescription. After this time period, a prescription will be required for further treatment. It is your responsibility to ensure a valid prescription is on file. Exception: This does not hold true for those insured through Medicare, Workers Comp, or No Fault.
* Prescription Renewal Policies - It is the responsibility of the patient to obtain all required prescriptions in order to continue physical therapy. Regardless of your insurance plan, we require you to have a valid prescription on file, once the "Direct Access" period has been met.
